Instructions: Unit 2 Self- Assessment Background: The PAR-Q and Health Status Questionnaire are tools used to determine if exercise is safe and appropriate for a client. For this assignment you will conduct a personal assessment on yourself regarding your exercise and fitness needs and your health history and risk factors. Instructions: To complete this assignment, first complete the PAR-Q on pages 19 and 20 as well as the HSQ on pages 22 and 23 in your FPH textbook. Based on that information answer the following questions: Part I: Health History and Risk Status: • Did you answer “yes” on any questions on the PAR-Q form or Sections 2 and 3 of the HSQ? If so, which questions? • Based on your responses, are you considered high or low risk? Do you require physician’s approval before beginning an exercise program, why or why not? Determining Fitness and Exercise Needs: • Describe the minimum exercise recommendations for you per the Physical Activity Guidelines for Americans based on your age and cohort. • Are you currently meeting the recommendations? If yes, provide a detailed discussion about your current exercise program. If no, provide a detailed discussion as to how you are falling short and why. • If you are not currently meeting the recommendations, what changes do you need to make to bring your activity levels in line with the Guidelines; be specific. If you are currently meeting the Guidelines, identify an area that you may like to target for improvement in your fitness program. • Project must be in APA format and utilize references.
