Examples of these models include the hospital at home model and the perioperative surgical home (PSH) model. The hospital at home model provides hospital-level care at the patient’s home, while the PSH model provides patient-centered care based on shared decision-making between practitioners and patients. The PSH model is team-based and is tied to delivering quality patient care at reduced costs (Chimento and Thomas, 2017).
